It's per year, but you can tie 2 years together.
Meaning you can have the last 5 months (150days) of one year and the first 5 months of the next year consecutively.
I did it myself three times, like that, until I finally got my business visa to stay.
And no you don't need to leave the country, as the two can run consecutively, giving you 300days spread across two calendar years (jan-dec).
So when you arrive: whatever remains this year+ 5 months next year

Glad to help, I'm sure as you stay and get to know Nepal you will find many confusing scenarios as this one :-).
This one I have gone through myself often enough, it's as I said (tested/proven)
When you arrive at the airport/border buy 3 month visa for $100, it'll never be cheaper than that again for you.
After, you will pay $60 per month to renew. A highly outragous price, but what to do...
